Requirements:
1. Must be a senior care service or facility that serves senior citizens in Dallas, Rockwall Kaufman or Collin Counties
2. Must be licensed by the state of Texas with an appropriate license in good standing and appropriate Texas or DAD's license must be up to date.
3. Must agree and allow Dallas Senior Connection to verify all information
given.
4. Must show or have some proof or plan in place that goes above and beyond normal licensing requirements. Examples: lower patient to staff ratio, extra training, different way of assuring quality, these are just several examples you may feel your facility offers something special.
